This script was copied from the CPAN CGI.PM documentation topic A complete example of a simple form-based script.

The script was modified to add Style, the Camel icon, the menuBar, and these comments were changed to note where I got the original script and provide the links.

I also added a call allowing this program to display it's program source with the link /Forms-Test?Source, display the changes I made with the link /Forms-Test?diff and display the revision log with the link /Forms-Test?rlog
